How Do I Join?

Our CarePartners program is delivered in partnership with health plans and primary care providers (PCPs). If you live in Pennsylvania and are eligible to enroll with us, ConcertoCare or your health plan will reach out to you directly.

ConcertoCare's CarePartners program offers a full array of personalized in-home medical care and other supports designed to keep you healthy and live life on your terms. We deliver our care services in coordination with your existing PCP and with specialists.

 Here's what you can expect when you first enroll in our program:

  1. We reach out to you to confirm your interest in the program – because we work with your health plan, there are no complicated forms to fill out
  2. We schedule your first in-home visit 
  3. Our provider visits you at your home and conducts a full evaluation of your health needs and personal goals
  4. We develop a care plan that is customized to you 
  5. You are now a ConcertoCare member – we regularly help you manage your healthcare needs throughout the year, and you have access to the full set of our services if something comes up
